DEATH

In the event of a participant’s death, the participant’s spouse or other beneficiary must submit a Death Benefit Application Form and a certified copy of the participant’s death certificate to the Fund Office within [XX] days after the participant’s death. In the event of a dependent’s death, you may need to update your Beneficiary information.
